KHS Student Action Plan: Grade 12
- Take the right courses to earn your Hawaii High School Diploma.
- Take challenging courses and consider going for an Honors Recognition Diploma.
- Attend school regularly and get the best grades possible.
- Participate in extracurricular activities and volunteer opportunities. Include all activities in your Personal Data Sheet.
- Record your extracurricular activities and volunteer work in your resume for your Personal Transition Plan (PTP).
- Meet with your counselor to make sure you are on track to graduate from high school and reach your college & career goals.
- Take the SAT in early Fall (August , September or October)) if you need to improve your score of if you did not take it your Junior Year. Register on line at sat.org/register Fee Waivers are available for Seniors who qualify for free or reduced lunch. Visit the Hawaii DOE website for eligibility information and the on line application for the free and reduced lunch price program.

- Register for NCAA Eligibility and/or NAIA Eligibility if you want to play Division I or Division II college sports. Make sure your end of Junior transcript has been sent to NCAA and/or NAIA if you did not do it at the end of Junior Year. Fee Waivers for NCAA and NAIA registration are available for Seniors who qualify.
- Take the ASVAB if you are interested in joining the military and you need to improve your score if you did not take it your Junior Year. It is offered at Kohala High School one time per school year (November).
- Apply for colleges by the deadlines: Early Action and Priority Deadlines provide the most financial aid opportunities.
- Send your Official Transcripts to the colleges or scholarship organizations that require them for the application. Complete the Kohala High Transcript Request Form and submit it to the Registrar's Office or email the Registrar for your transcript request. Be mindful of deadlines! It takes up to 5 days to process transcripts so submit your requests early. The cost is FREE. Click here for a transcript request form. Provide an envelope with the transcript request form.
- Understand how to pay for college on the College Board and Federal Student Aid websites
- Attend the Financial Aid Night / FAFSA Workshop during the 1st Quarter and a FAFSA Finish Event early in the 2nd quarter.
- Create your FSA ID and apply for financial aid with the FAFSA beginning October 1st.
- Apply for scholarships from the colleges you are applying to and from the Scholarship lists in this website.
- Request letters of recommendation for college admissions and scholarships based on your application needs. Check this out before you ask so that you Get Great Letters of Recommendation. Ask those that your wish to recommend you at least 2 weeks in advance by using the Kohala High College Recommendation Request Form or the Scholarship Recommendation Request Form
- Write your personal statement/college essay for college admissions and scholarships. Get tips from College Board and Purdue.edu
- Complete Personal Transition Plan (PTP) in lessons in your Advisory class. Keep in mind that the PTP is a 1/2 credit requirement for graduation and is due before the end of Semester 1. This is non-negotiable and is a Hawaii Department of Education graduation requirement.
- Go to College & Career Fairs (Fall and Spring), College Visits, and possible college field trips.
- Decide which college you will enroll and complete enrollment and college housing steps including paying any deposits required. See the School Counselor if you need help with enrollment steps.
- Submit the final transcript request for your transcript to be sent after graduation. The cost is FREE Click here for a final transcript request for graduating seniors only.
- If you plan to play sports in college, then send your final transcript to NCAA or NAIA depending on which organization the college you will be enrolled uses for eligibility. The cost is FREE but you must provide an envelope. Click here for a final transcript request for graduating seniors only.
- Apply for summer opportunities in your career field of interest or at the college you will be attending.
- Submit all college acceptance letters and financial aid/scholarship award letters to the School Counselor so that you can be recognized in the Graduation Program as a Scholarship Recipient and the college you plan to attend. Rise to the Challenge Cowboys!
